GUIDELINES TO FILL THE DOWNLOADED FORM

Form must be computer typed (fill it on computer)
Summer Programs form to be filled by hand
Must enter your name as mentioned on your CNIC
Attach required documents mentioned in the check list
Submit the form with documents to admission office
Download form on a PC or a laptop, it will not work on mobile
Form is non-editable MS Word document
If you have any difficulties in filling the form, you can do so at our campus where we will guide you through

REQUIRED DOCUMENTS (only for Diploma Programs & Short Programs)

• Copy of Matriculation certificate and/or mark sheet or of equivalent qualification

• Copy of Ninth grade mark sheet (for awaiting students)

• Copy of applicant’s CNIC (B-Form if applicant is under 18 years)

• Copy of parent’s / guardian’s CNIC

• Copy of Nadra COVID Vaccination Certificate

• Two passport size recent photographs
